Locations
CCP provides services from seven primary locations in the metropolitan Twin Cities area. They are located in Minneapolis, St. Louis Park, Plymouth, and Brooklyn Park. Each site is well equipped and staffed to meet the client's needs. The locations all have a skilled and experienced on-site manager to maintain our organization's commitment to quality, innovative services.
Transportation:
People come to our work centers from all over the Twin Cities area. Community Connections Partnership provides vans to assist in meeting transportation needs. We offer a pick-up and drop-off service for those who need assistance. Our van drivers are experienced working with people with disabilities. In some cases, direct care staff transport people in their cars. This helps to minimize the difficulty of transitioning from home to work and vice versa that can occur for some individuals.
